the health appraisal questionnaire within the new patients forms on our
website. Once completed, call the clinic at 630-858-9900 and we will schedule
your initial appointment with Dr. Kelenyi.

What Is Included?
- A consultation with
Dr. Kelenyi to discuss your problems and concerns.
- A complete case
history and questionnaire.
- A dietary and
nutrition evaluation.
- A complete analysis of
dietary habits.
- In addition we are going to analyze your current
supplements, herbs, natural medicines, botanicals, and homeopathic's to determine whether
they are beneficial or actually detrimental to your condition.
- We will also have you
provide a copy of your medical records and lab tests and we are going to go
over them in a detailed manner.
- We are going to
determine whether you have had the appropriate lab tests ordered, and make a
decision of what further testing is required.
- Implementation of any
immediate beneficial changes we can make based off of previous labs and your
history.
